Long Term Conditions questionnaire

Andrew MacLean of Partnerships For Wellbeing is working on a project for NHS Highland (which covers the Argyll and Bute area). Part of the project is to identify the community groups which are doing work in the area of supported self-management for people with long term conditions. These conditions could be such as MS, Arthritis, Diabetes, Epilepsy, etc.  There is no definitive list and it could mean anything which a person suffers from for over a year.  He is looking to identify groups that are open to/aimed at people with long term conditions and/or do something which people with long term conditions could take part in.  This could range from swimming classes to shinty to drama to reading, anything really.

If you would like to help with Andrew’s research could you please complete the questionnaire below: or alternatively send him an email with the following information:

  •  name of the group
  •  when/where it meets
  •  what it does (e.g. physical activity, social events)
  •  is it aimed at a specific group of people e.g. elderly stroke sufferers, young MS sufferers, etc)
  •  contact details for who runs the group.
